Navigation Care System Support
Our Navigation Care System provides practical, person-centred support to help individuals understand their rights, access services, and feel more confident when dealing with complex systems and official processes. Support includes:
- One-to-one “What am I entitled to?” guidance sessions to help individuals understand available benefits, support options, and services they may be eligible for
- Signposting and referrals to relevant local organisations, community groups, healthcare providers, advocacy services, and specialist support agencies
- Help understanding letters, forms, decisions, and other official correspondence from councils, healthcare providers, the DWP, housing services, or other agencies
- Support with navigating health, social care, housing, welfare, and community support systems
- Assistance identifying next steps and accessing appropriate services based on individual needs and circumstances
- Information and guidance delivered in a clear, accessible, and supportive way to reduce confusion and improve confidence
- Ongoing support to help individuals stay connected to services and avoid falling through gaps in provision
