Comprehensive Support Assessment & Action Plan

A dedicated 1-hour consultation with the individual and/or family to gain a thorough understanding of needs, goals, and current challenges. Following the consultation, you’ll receive a personalised written action plan outlining available support, tailored recommendations, and clear next steps.

The service includes:

  • Personalised written support plan and recommendations

  • Assistance with Attendance Allowance and Pension Credit applications

  • Support with social care referrals

  • Sourcing and coordination of home care solutions, assistive technology, and other support services

  • Signposting to relevant local and national resources

  • A 4-week follow-up check-in to review progress, address any challenges, and update recommendations as needed
